An annual physical form is required for all student-athletes. All physical forms must be dated AFTER MAY 1ST, 2024 in order to meet the medical requirement for the entire 2024-25 School Year.

Cost: $25, Cash or check ONLY, no card or Venmo payments. Checks should be written out to Eanes ISD.
Each athlete's physical will be scanned into our system and checked off for participation on rankone. They will also be sent home with a hard copy for records and summer camps as needed.
If student-athletes choose NOT to complete the physical at Westlake on one of these nights, they must submit physical forms via email to a staff athletic trainer. All physical forms must be dated AFTER MAY 1ST, 2024 in order to meet the medical requirement for the entire 2024-25 School Year. Once received, we will review individual forms and ensure that all required fields are filled in correctly for safe participation. You will receive a response via email if it was accepted or needs revision.
Also, complete your online UIL/Eanes forms via rankone: eanesisd.rankonesport.com
- Rankone forms will go live on May 1st for the next school year eligibility
- You must have a current student ID for sign-on/registration to rankone (student ID is assigned as your lunch number and skyward information)
- These forms must be digitally signed by a parent
- Nothing can be uploaded to rankone and there is no need to download these forms
- If all forms are correctly signed you will see a “yes” indicated next to it
- Once an athlete/parent has successfully sent a completed physical form and signed all digital documents on rankone, the profile will show GREEN, indicating full participation
